Your first appointment at our office is designed to meet you and discuss your orthodontic concerns. This visit is complimentary. Dr. Ulrich will perform a clinical examination, and the information gathered during this exam will be helpful in developing a custom treatment plan for each patient.	Please help us be prepared for your first appointment by completing this Patient Information and Medical History Form. Download and print the forms, fill them in, and return them to our office. If orthodontic treatment is indicated at this time, we will discuss your treatment with you in detail, including the cost for your particular case, and available financing options. *Beginning Treatment with our office* We will arrange a time for you to have beginning orthodontic records taken. The films we take are an important part of your orthodontic treatment. From these pictures we can prepare a plan for treatment that is unique to you alone. Allow several days for the processing, review, diagnosis and treatment planning of your films. Your first visit to place your braces will be scheduled accordingly. *Appointments* The appointment when you get your appliances (braces) usually takes one to two hours. You will then see us at regular intervals 4 to 8 weeks apart for an adjustment, which takes 20 to 40 minutes. Our regular office hours are Monday to Thursday from 7:45 am to 12 noon, and 1:30 pm to 5 pm. Friday’s from 7:45 to 12 noon. Patients are seen by appointment only. We make every effort to be on time for our patients, and ask that you extend the same courtesy to us. If you cannot keep an appointment, please notify us immediately. Cancel only if it is an emergency. It is difficult to reschedule you since most appointments are made 4 to 8 weeks in advance.
